Objectives: This study assessed the effect of anatomical factors on the visibility of the mandibular canal (MC) roof on panoramic radiographs (PRs) using cone-beam computed tomography (CBCT).
Materials and Methods: This cross-sectional study analyzed 92 quadrants in CBCT scans of 360 patients. MC roof visibility was evaluated at the first premolar, first molar, second molar, and third molar sites first on both CBCT and PRs. Its association with age, gender, tooth site, gray level, distance from the MC roof to the alveolar crest, thickness of the buccal cortical plate, MC roof-inferior mandibular cortex distance, dental status, total buccal bone thickness, and MC diameter was analyzed using independent t and Chi-square tests (alpha=0.05).
Results: Dental segments of 176 males (48.9%) and 184 females (51.1%) with a mean age of 47.34±13.66 years were assessed. MC roof visibility was significantly associated with age, gender, tooth site, gray level, distance from the MC roof to the alveolar crest, and thickness of the buccal cortical plate (P<0.05).
Conclusion: CBCT showed superior MC roof visualization compared to PR. Visibility was higher in younger patients, males, third molar regions, higher gray levels, greater alveolar crest distance, and thicker buccal cortical plate. Poor MC roof visibility may reflect low bone density and should be considered in orthodontic and surgical planning.
